Healthcare: repeat examination

Hi, I have the attached model. I want to have the patient repeat the examination 3 times before release. I have used a label as a counter to check If the value is 3 then we will release the patient. But the model, did not work. During the second round, the activity did not get to continue. Please advise what happen.
