how to create a child patient accompanied with the parents ?

1) I want to create a child patient accompanied with parent and how can I do it?

2) When the doctor examine the child patient, only the child separated from parent acquire the exam bed (or lie in exam bed) but when move to other exam area, the child with parent should escorted by nurse. How can I do it?
