A 3D spline curve is not possible. As you describe there is a linear rising or falling feature by different z- location values for start and end.
You can place a conveyor on a visual plane and rotate this plane to get at least a motion of flowitems in any angle, but you still need then a suitable shape fitting to this motion.

Here's a proof concept how this could work. Though as you can see when looking at the triggers of the conveyors, there is quite a bit of work involved to get the motion (of just this specific item size) to look smooth.

If you use the legacy conveyor you can rotate multiple sections (of one conveyor) as your diagram shows.
