Essentially I am trying to just move an item (sphere) into the box once it reaches a certain decision point. For some reason when I use "move item" to put the sphere into the box at this point, the sphere becomes offset by a bunch.

Your Move Object activity is working and the sphere becomes a subnode of the box. However, it is bizarre it is offset so far away. You can fix this by simply adding a Change Visual activity after the Move Object to update the sphere's location to the box's location. This makes the model look like this:
