I have a model where I have 4 flowitems, combine 1 and 2, and 3 and 4 are combined separately, but using the same combiner. Is it possible to use a "combiner" for different combinations?

You may want to check out this previous answer that talks about different combine operation strategies with both process flow and object based approaches.
Cristian,
The common method for doing this is to use the pick list option "Update Combiner Component List" in the entry trigger of the combiner. This will allow you to pull different quantities from the input ports 2+ based on the itemtype of the item pulled from port 1 (the first item to be pulled).
This solution may not be the perfect answer for your problem but it will be the best starting point for you.
I hope this helps,
Brandon
