How would I setup seeing number of patients seen per hour by the MD

Do you have a model you could post? I suggest thinking about what indicators there are of "seeing a patient". For example, is it every time the MD is acquired? Is it every time an MD travels to a certain location? Is it every time a token passes through a certain activity?
Once you know what to track, you can build a stats collector that listens to that object or instance.
