In regards to your question about having 50 patients arrive, it looks like you need to edit your Arrivals flow. By default the setting are to have only one person come in, so if you are only seeing one person it is because you need to edit the Source in your arrivals flow. In the default source, you can put in a time range and the number of patients you want to enter in that time range. You can space them evenly or randomly over the time interval you specify. This setting is found at the bottom of the "Arrivals - Source Arrivals" window. If you would like it to repeat daily, you can turn on the Repeat Interval found right above the table.

If you want to have patients follow different activity paths in the model, I would recommend adding it to your Patient Flow. Making dynamic decisions, like sending a percentage of patients through a certain set of activities, is handled using a Decide Activity. The Decide Activity is found in the Basic section of your Process Flow Library. This activity is one of a small number of activities that can have more than one connector out of it. The Decide Activity has a pick-list option for "By Percentage" where you can say what percent go out what connector. This is what I would use and route patients to different activity sets. This same activity could also be used in the Arrivals Flow if you would prefer to create two different patient flows for the different patients steps. You would just want to add it in before the Create Patient Activity.
