The issue is that when you merge slug lanes you need to have inline transfers between the slug lanes and the merge lane. The easiest way to do this, as Sam suggested, is to have curved conveyors between the slug and merge lanes. Otherwise you need to click on the transfers and set them up to be inline transfers. See the answer to this question Merging slug lanes without curved conveyors for more information.
I look, but I didn't find the option to set up a transfer as an inline one.
Read the answer on the other question I linked to. You have to change the angle at which a transfer is considered inline, or just add small curved sections which looks better visually.
I saw it, but there is says how to change the characteristics of an inline transfer, but not how to turn a transfer into an inline one.
Conveyors sometimes have some strange things that happen. Basically what happened is a little timing quirk where one item is coming in at just the wrong time as the slug from that lane is releasing. Since the item is not part of the slug, but is timed to line up with the slug's release, some how it gets the conveyor stuck, where the lane is still blocked as if it's reserved for that slug, but there is no longer a slug there to release.
One simple solution is to create small "connector" pieces of conveyor between the slug lanes and the main lane. Such as this:
