Staff Assignment logic help

I will describe my difficulty in the body of this mail: The basic setting of the model.

There is a stream of patients of 4 categories coming in with different percentages( say 25-25-25-25% in each type). There are 2 different groups of doctors( A & B with 5 members in each group). There are 3 facilities where patients are assigned on first come first serve basis between those facilities based on their capacities( i.e the new patient will be assigned to the facility with least occupied beds)

1. I want to assign patients coming in at 1:1 ratio between these doctor groups for treatment. I know there is the 50-50% option in staff requirement stage.

But I would like to know how i can maintain a strict 1:1 ratio.

2. When i gave the condition as 50-50% split of patients between both doctor groups- in group selection policy - I tested Utilization vs Proximity condition. When I changed that the load on different members got divided unevenly while using proximity condition. With some having to deal with maximum load. So, is there a way i can define an upper limit on number of patients a doctor can treat in one go. Like define a threshold value- for example: Say a doctor from group A got treated 5 patients one after the other then the next patient would not be assigned under him but to some one else who is closer or from same group - not utilized so far?

I think you will appreciate the answer I gave in this post, because the situation sounds very similar. You'll want to check out the example model I attached to my answer in that post named "Equitable Nurse Assignments Based on Acuity Workload.fsm". My example model assigns patients to the nurse with the lowest current workload where "workload" is calculated by summing up the acuity levels of all patients who are currently assigned to the nurse, but you could modify the approach to not consider the patient's acuity, but only sum up the number of patients currently assigned to the nurse/doctor. Let me know if this isn't what you're looking for, and I'll try to answer your question directly. If you have an example model you're working with, please send it along as well, because I'm not sure how the 3 facilities are being shared among the doctors/patients.
