How can you get random patients to be wearing surgical masks?

I was wondering if there's a way to put surgical masks on random patients. The "Change Visuals" activity offers "Facial Hair/Mask" but I'm not seeing any patients arriving wearing masks. I read the question regarding patients with gowns but I don't understand the syntax for the "Index" field.

The index lets you choose one of the things in the list of choices that each shape has. For example, the Male Person/Staff shape has 4 different choices for Facial Hair / Mask. 1 would put a beard on the man, a 4 would put a Mask on them. The Female shape only has 1 choice, so a 1 would be a mask.

I've attached a sample model that puts masks on 50% of the arriving patients. I added a Change Visuals option and configured it like this:
