How do I export a table with all the label values for each patient?

In my model, I am using the labels to keep track of what procedures each patient undergoes during their appointment. I want to export a table with each patient's label values, so I know for example, that Patient 1, has Biposy=1, Catheter = 0, Surgery Scheduling = 1, etc.

You can use the Statistics > Output Raw Data menu option. Be sure to check the User-defined Labels box as shown below.
